Plain-language privacy information

Your information.
Handled with care.

This page explains what Doing Life Today collects when you apply to become a Community Connector and how that information is used.

Last updated: September 14, 2026

What we collect

The application asks for your name, email address, country, city or town, state or province, ZIP or postal code, your connection to Parkinson’s, whether you are already in The Club, the ways you may like to help and your optional reason for wanting to become a Connector.

We also record your confirmation that you understand the Connector boundaries and that DLT may contact you about the Community Connector program.

Please do not include medical or private information

We do not ask for diagnoses, symptoms, medications, treatment details, medical records or anyone else’s private information. Please do not include those details in the optional written answer or in an email about your application.

Why we use it

Authorized Doing Life Today team members use your information to review your application, contact you about the result, support onboarding and training, administer your participation and keep an appropriate program record.

Your application information is not used to make medical decisions or provide medical advice.

Where it goes

When you choose “Submit application,” your answers are sent to Doing Life Today’s protected operations system. They are available only to authorized people and services involved in operating the Community Connector program.

If direct submission is unavailable and you choose the clearly labelled email option, the information is sent to help@dolifetoday.com instead.

How long we keep it

DLT keeps application and participation records only as long as they are reasonably needed to review applications, operate the program, provide support and meet recordkeeping obligations.

Your choices

You may stop before submitting the application. Your answers are not sent to DLT until you choose “Submit application.” A temporary draft may remain only in this browser on this device so you can return to the form.

You may ask DLT to review, correct or remove information connected to your application, subject to any information DLT needs to retain for legitimate program or recordkeeping purposes.
